All information shared in private consultations and workshops are meant to facilitate health, balance, and the well-being of an animal and human. All written materials, lectures, discussions, question and answer sessions, references to other modalities, products, and practitioners are means to provide information through consultations and workshops. At no time is the information provided meant to be a replacement for traditional medical care, veterinary treatment, or a medical diagnosis. Morgan Mellick and The Animal Alchemist does not diagnose medical issues or offer medical advice for an animal or human. Please consult a licensed medical doctor or veterinarian for medical care.

I have read and understand the above statement. I understand that Morgan Mellick and The Animal Alchemist is not a veterinarian or medical professional and that the consultation/treatment/therapy/advice is not to be construed as veterinary or other medical advice.

By booking with Morgan Mellick and The Animal Alchemist, I represent and warrant that I am the lawful owner, legal guardian, or fully authorized agent of the animal(s) for whom the booking is made. I assume full legal and financial responsibility for the animal(s) and affirm that I have the complete authority to request, consent to, and authorize all consultations, workshops, or services provided by Morgan Mellick and The Animal Alchemist.

Morgan Mellick and The Animal Alchemist focuses strictly on facilitation, well-being, and education. Morgan Mellick and The Animal Alchemist does not provide legal advice, nor do we accept cases, provide consultations, or issue documentation intended for use in active legal disputes, custody battles, or lawsuits regarding your animal.

I hereby release and hold harmless Morgan Mellick and The Animal Alchemist from any and all liability, claims, or demands due to accident, injury, or damage to self, animal, and/or personal property, or from loss of property resulting during or after any consultation, workshop, or engagement, including the following of any suggestions, information, instruction, or advice given by Morgan Mellick and The Animal Alchemist.

Furthermore, I acknowledge that the services and advice provided rely entirely on the accuracy and completeness of the information I provide. I expressly agree that Morgan Mellick and The Animal Alchemist is not liable for any negative outcomes, errors, or disputes resulting from information that I have withheld, or that is incomplete, inaccurate, or provided under false pretenses. I assume full responsibility for all risks associated with the completeness and truthfulness of the context I provide.

Cancellation Policy: All bookings, consultations, and workshop registrations are strictly non-refundable. If you need to change your appointment, you may reschedule your session one time, provided that you submit your request in writing at least 48 hours prior to your originally scheduled start time. Cancellations or rescheduling requests made less than 48 hours in advance, or any subsequent requests to reschedule a second time, will result in the forfeiture of your booking fee.

Exception for Specialty Services: Any specialty services, custom packages, or consultations booked exclusively through direct inquiry and custom scheduling are strictly final. Because these unique sessions require direct clearance to book, are often of a time-sensitive nature, and are not available for standard online scheduling, they carry a strict zero-cancellation, zero-rescheduling, and zero-refund policy for any reason under any circumstances. For custom packages, while non-refundable, rescheduling options can be discussed due to our ongoing work.

I as owner, or agent of the owner, agree to have this consultation for my animal and/or myself and by my payment of and participation in said consultation, give my consent and agree to adhere to all terms of this agreement and release form.
